Download Russian visa application form sample

The Ministry of Foreign Affairs of the Russian Federeation has opened an official Russian visa application website http://evisa.kdmid.ru/. This website contains the official Russian visa application form. For some countries (for example for the US and the UK) a Russian visa application must filled in on the above said website only.

For all other countries you can download a Russian visa application form samples from this page. Actual and updated Russian visa application forms can be found at the relevant Russian Embassy or Russian consulate websites.

Please note that all downloaded Russian application forms must meet the following requirements:

1. The Russian visa application form must be filled out on computer or using a black pen or typing machine in capital letters. Using ink pens is not allowed. Writting must be legible.
2. Questions 1-7 of the application form must be answered by all applicants for Russian visa
3. Question 8 of the application form is to be answered if you know what kind of visa to Russia you are requesting
4. Question 12 of the application form is to be answered, if you have already visited Russia
5. Questions 13-14 of the application form must be answered by all applicants for Russian visa
6. Question 15 of the application form is to be answered if you have medical insurance valid in Russia. You should put policy number and the name of insurance company.
7. Question 16 of the application form is to be answered if you are travelling to Russia with the children included in your passport. A child having own passport must apply for a Russian visa separately.
8. Questions 18-21 of the application form must be answered by all applicants for Russian visa
9. Downloaded Russian visa application form have to be printed on both sides of a single sheet of paper.
